Request for Incident/ Accident Report Form

A copy of an Incident/Accident report can be requested via email for any resident of Weathersfield Township.

We request that insurance companies and non-residents can either request a report via mail or can pick up a copy at our Administration Building during normal business hours. There is a charge of $.25 for first three (3) pages and $.10 for each additional page.
